Important information regarding AGENT ORANGE
The Government of Canada is offering a one-time tax-free ex gratia payment of $20,000.00 for eligible individuals who served/trained/worked/lived in CFB Gagetown during the time frames of June to September 1966 and June to September 1967. These individuals must also meet certain medical and geographical criteria.
The deadline to apply is April 1, 2009. The following are documents which can be printed from the Veterans Affairs website required to submit a claim:
*Application for ex-gratia payment
* Physician's statement for ex gratia payment
* Canadian Forces Authority to Release Service Information
* Authority to release personal information ( this is only required if someone has Power of Attorney to handle your affairs on your behalf.)
Please note that the doctor completing the Physician's Statement must also attach medical documention supporting the diagnosis. This could include Pathology Report, Report of Operation etc. VAC will not re-imburse for any physician's fees charge for completing this medical questionnaire or providing supporting documention.
Attention Widows : Please note the following decloration must be included with your application on page 7 and signed and dated. This will allow VAC to access your late husband's DVA file and obtain the required medical information if it is contained within for the condition in which you are applying for. Please print this form off.
TO WHOM IT MAY CONCERN:
I GIVE AUTHORIZATION TO DVA TO USE MY HUSBANDS VA FILE FOR THE PURPOSES OF OBTAINING MEDICAL, SERVICE AND ANY OTHER INFORMATION REQUIRED FOR THE COMPLETION OF THESE DOCUMENTS.
